How to Apply
An explanation of application procedures is outlined below. The administration of The Lodge on Lafayette and Rose Haven are the same, with the office located at the front entrance of Rose Haven at 1500 Franklyn (under the canopy).

David and Coleen Yearian are the owners and administrators of the Lodge. A meeting between one or both should be arranged to begin the application process. Other professionals are available for questions in their absence.

A simple application will be completed at the time of the first visit. Filling out an application does not further obligate you. Only generic information shall be obtained in this application step.

A showing of the unit will be arranged, either on the day of the application or later, if desired.

If the applicant(s) desires an opportunity for an apartment, a physical examination by your own personal physician is required. We will provide your physician with an examination form which, upon completion, must be reviewed and approved by the Lodge Medical Director, Administrator and Nursing staff to assure the appropriateness of the applicant.

After a review of all information, a personal visit will be arranged to discuss the outcome of our review and, if appropriate, the resident agreement will be reviewed.

Deposit will vary as to the fee option agreed upon.

Prior to admission, the remaining portion of the fee is paid. The key to the unit will be given to the applicant at that time.
